Violation and Irregularity Reporting Form

The Abuse Reporting Form allows you to submit information about improper and undesirable behaviors that result in violations of applicable laws, internal regulations, and ethical standards.

Has the incident been previously reported to another authority or supervisor?*
Do you wish to remain anonymous?*
If "yes" is selected, the personal data provided above will not be used during the proceedings.

A report of misconduct may only be made in good faith.

Knowingly making false reports of misconduct is prohibited.

If, as a result of a preliminary analysis of the report or during the explanatory proceedings, it is determined that the misconduct report contains knowingly false information or that the truth was deliberately withheld, the reporting person, if an employee, may be held disciplinarily liable in accordance with the provisions of the Labour Code. Such conduct may also be classified as a serious breach of fundamental employee duties and, as such, may result in termination of the employment contract without notice.

In the case of a reporting person providing services or delivering goods to the Office/Municipality under a civil law contract, the determination that a false report of misconduct was made may result in the termination of that contract and the definitive end of cooperation between the parties.

Regardless of the consequences indicated above, a reporting person who knowingly submits a false report of misconduct may be held liable for damages if the false report results in harm to the Office/Municipality.
